If, for any reason, you are unable to keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments canceled less than 24 hours before the scheduled appointment time.

Insurance & Billing

Our office accepts a variety of HMOs, PPOs, and other health plans. Please call our office to verify that we accept your insurance carrier. Payment of co-pays and deductibles is expected at the time of service unless prior arrangements have been made.

We accept MasterCard, Visa, American Express, Discover, cash, or check. Please note that if you are paying for services by credit card, you will be charged a 3% processing fee. This charge does not apply to debit card, FSA, or HSA transactions.
